Bone Grafting

MOST COMMON QUESTIONS ABOUT BONE GRAFTING

WHAT DO YOU USE BONE GRAFT WITH?

MOST OFTEN WE USE HUMAN TISSUE THAT HAS BEEN PROCESSED TO THE MOST STRINGENT OF FEDERAL GUIDELINES AT THE MIAMI TISSUE BANK. BOVINE (COW) TISSUE IS USED FOR THOSE WHO REQUEST IT. WHEN ONLY THE BEST WILL DO, OUR PREMIUM PRODUCT IS CALLED ACCELL BY KEYSTONE DENTAL. DUE TO THE HIGHER COST, THERE IS A SURCHARGE FOR THIS MATERIAL. ACCELL UTILIZES PROTEINS AND GROWTH FACTORS AND RESULTS IN MORE BONE GROWTH PER SITE. LEARN MORE ABOUT THIS EXCITING NEW MATERIAL AT WWW.KEYSTONEDENTAL.COM. CLICK ON “IMPLANT SYSTEMS” THEN SCROLL DOWN TO “ACCELL”. WE HAVE BEEN PROVIDING GRAFTING PROCEDURES FOR MORE THAN 15 YEARS AND NEVER HAD A PROBLEM. ANTIBIOTICS ARE PRESCRIBED TO MINIMIZE THE CHANCE OF INFECTION AS WE DO FOR MOST OF OUR SURGERIES.

WILL IT HURT?

SMALL GRAFT SITES DO NOT HURT AT ALL. LARGER SURGERIES MAY RESULT IN POST-OP SORENESS THAT MAY LAST UP TO TWO WEEKS AND IS CONTROLLED WITH MEDICATION.

WHY DO I NEED IT?

WHEN TEETH HAVE BEEN MISSING FOR 6 MONTHS OR MORE THE BODY SHUTS DOWN BONE FORMATION WHERE THE TOOTH ROOTS WERE, AS IT IS A WASTE OF ENERGY FOR THE BODY TO MAINTAIN BONE THAT IS NOT BEING USED TO HOLD IN TEETH. YEARS LATER WHEN A PATIENT DECIDES ON DENTAL IMPLANTS THE AVAILABLE BONE IS OFTEN INSUFFICIENT FOR ANY SIZE IMPLANT.

WILL I NEED A SECOND TREATMENT?

GRAFTING PROCEDURES ALWAYS RESULT IN NEW BONE BUT SOMETIMES NOT ENOUGH BONE IS GENERATED TO SUPPORT THE LARGER IMPLANT THAT IS PREFERRED FOR LONG TERM FUNCTION OF THE TEETH ON TOP OF THE IMPLANT.

HOW LONG DOES IT TAKE BEFORE AN IMPLANT CAN BE PLACED?

AT LEAST 4 MONTHS IS REQUIRED - 6 MONTHS IS BETTER IN YOUNG, HEALTHY NON-SMOKING INDIVIDUALS. OLDER PATIENTS WITH HEALTH ISSUES MAY REQUIRE A YEAR FOR THE GRAFT MATERIAL TO TRANSITION INTO HEALTHY DENSE BONE. SMOKERS UNDER 50 DO NOT GRAFT WELL AND MAY REQUIRE ADDITIONAL TREATMENTS, AT ADDITIONAL EXPENSE, TO GENERATE ENOUGH BONE FOR IMPLANT PLACEMENT. SMOKERS OVER 50 TEND TO HAVE OTHER HEALTH PROBLEMS AND ARE SELDOM GOOD CANDIDATES FOR BONE GRAFTING OF ANY KIND. SOCKET GRAFTING AFTER TOOTH EXTRACTION, TO SAVE AS MUCH OF THE RIDGE AS POSSIBLE, IS THE ONLY GRAFTING PROCEDURE RECOMMENDED FOR THIS GROUP OF PATIENTS.

IS THE PROCEDURE GUARANTEED?

IT IS UNETHICAL FOR A MEDICAL OR DENTAL SURGEON TO GUARANTEE ANY PROCEDURE AND OF ALL THE PROCEDURES WE PREFORM IN DENTISTRY, 'PARTICULATE GRAFTING' IS PROBABLY THE LEAST PREDICTABLE OF ALL. 20 YEARS OF GRAFTING HAS TAUGHT US THAT ABOUT 60% OF OUR PATIENTS GENERATE ENOUGH SOLID BONE FOR THE PRESELECTED IMPLANT. 35% REQUIRE A SECOND PARTICULATE GRAFT AND 5% REQUIRE A 3RD PROCEDURE. IF ALL OUR GRAFTING PATIENTS WERE PERFECTLY HEALTHY NONSMOKERS UNDER THE AGE OF 50 OUR SUCCESS RATE WOULD BE 95%. UNFORTUNATELY THIS IS NOT THE GROUP WHO REQUIRE GRAFTING PROCEDURES THE MOST. THE MAJORITY OF THE PATIENTS WHO NEED OUR HELP REBUILDING THEIR JAWS AND REPLACING TEETH ARE OLDER INDIVIDUALS, OFTEN WITH HEALTH PROBLEMS. ALSO, THERE IS A LIST OF DISEASES THAT CONTRA-INDICATE (NOT RECOMMENDED) IMPLANT OR GRAFTING PROCEDURES OF ANY KIND.

ARE THERE OTHER KINDS OF GRAFTING BESIDES PARTICULATE?

YES, (1) THERE IS BLOCK GRAFTING (TAKING BONE FROM OTHER PARTS OF THE BODY), ALSO KNOWN AS MULTIPLE SURGICAL SITE GRAFTING; (2) FULL ARCH GRAFTING WITH TITANIUM MESH - A 2 STEP SURGERY. THESE PROCEDURES MAY BE PERFORMED IN A HOSPITAL SETTING; WE REFER THESE PATIENTS TO DENTAL SPECIALISTS (ORAL SURGEONS) AND THEY WILL DETERMINE WHERE THE SURGERY WILL TAKE PLACE.

WHEN IS THE BEST TIME TO BONE GRAFT?

IF YOU THINK YOU MIGHT BE INTERESTED IN IMPLANT DENTISTRY, THE BEST TIME TO GRAFT IS THE DAY YOU HAVE A TOOTH EXTRACTED. SOME CASES WORK OUT BETTER IF GRAFTING IS PERFORMED A FEW WEEKS AFTER EXTRACTION.
